Advance Care Planning 101 Make a plan, together

Health emergencies can happen to anyone. We want you and your loved ones to understand your wishes. Having an Advance Care Plan (ACP) filled out and registered is one of the ways to make your wishes known to a hospital and family. 

During this workshop we will discuss what an Advance Care Plan is, how to register it, and why it is important for anyone over 18 years old. Between 2 -3 pm you will have the opportunity to work one on one with a trained Advance Care Planning Facilitator to fill out your own.  

The workshop will include information on:

  • Who needs an Advance Care Plan
  • What you need to know about Advance Care Planning
  • How to fill out an Advance Care Plan
  • Finished and registered Advance Care Plan

ACP Myths and Facts

Myth: Once I write down what I want, I cannot change it.

Fact: Your Advance Care Plan can be changed at any time. It is recommended to
review and make any changes at least once a year.

Myth: Only people only 50 years need to have an Advance Care Plan.

Fact: Medical emergencies can happen to anyone. Once someone turns 18 years old
it is important to have an Advance Care Plan in case of an emergency.

Myth: I have already filled out this document with my lawyer.
The hospital does not need to have it.

Fact: Your document must be filled out and uploaded to the
US Advance Care Plan Registry for a hospital to be able to
honor your wishes. If you have already filled it out
but are not sure if it is uploaded in the registry,
